The work ensures that individuals and families receive the guidance they need to access community resources. As a County Caseworker 2, you will perform the following duties: 1. Service Location: Assist participants and families in finding, arranging, and obtaining services identified in the Individual Support Plan 2. ISP Coordination: Work with participants, families, team members, and providers to develop and maintain the Individual Support Plan 3. Service Monitoring: Maintain regular contact to ensure all supports are delivered according to the participant plan 4. Information Support: Provide guidance to help participants choose whether to direct their own services 5.
