PRN Licensed Vocational Nurse (LVN) – Senior Wellness & Client Education

ComForCare Home Care – North Austin Position Type: PRN / As Needed

ComForCare Home Care – North Austin is seeking a warm, compassionate, and outgoing Licensed Vocational Nurse (LVN) who enjoys working with older adults and building meaningful relationships with clients and families.

This is not a traditional bedside nursing position. We are looking for someone who enjoys socializing with seniors, checking in on their well-being, providing education, identifying changing needs, and helping clients remain safe and independent at home.

Key Responsibilities

  • Conduct in-home wellness and client check-in visits.

  • Build trusting relationships with seniors through conversation, education, and ongoing support.

  • Check on clients' general well-being and identify changes or concerns that may need additional attention.

  • Educate clients and families on fall prevention, home safety, mobility, nutrition, medication awareness, and healthy aging.

  • Help clients and families identify resources that can support their goal of aging safely in place.

  • Review client needs and communicate observations or concerns to the appropriate member of the care team.

  • Support clients and families during changes or transitions in care.

  • Accompany clients to medical appointments when requested and help them understand follow-up instructions.

  • Communicate and coordinate with families, caregivers, physicians, home health, hospice, therapy providers, and other healthcare professionals as appropriate.

  • Provide caregiver education and support within the LVN's permitted scope of practice.

  • Maintain accurate and timely documentation of visits and observations.

  • Serve as a friendly, knowledgeable resource and advocate for our senior clients.

Qualifications

  • Current, unrestricted Texas LVN license.

  • Experience working with seniors or geriatric populations preferred.

  • Home care, home health, senior living, or community-based experience preferred.

  • Strong communication and interpersonal skills.

  • Enjoys talking with, educating, and building relationships with older adults.

  • Observant and able to recognize changes in a client's condition or needs.

  • Comfortable working independently in clients' homes and throughout the community.

  • Dependable, compassionate, patient, and professional.

  • Valid driver's license and reliable transportation.

Schedule

This is a PRN/as-needed position. You can build your own schedule.
