Job Description
The Home Health PT provides skilled physical therapy services to patients in their homes or hospice settings. Focus on improving mobility, reducing pain, and enhancing quality of life through individualized care plans.
Key Responsibilities:
Assess patient needs and develop personalized treatment plans. Deliver therapeutic exercises, gait training, and pain management techniques. Educate patients and caregivers on safe mobility and home exercise programs. Collaborate with interdisciplinary team members to ensure holistic care. Document patient progress and maintain compliance with regulatory standards.
Qualifications:
Licensed Physical Therapist (PT) in the state of Montana. Experience in home health or hospice preferred. Strong communication and time-management skills.
