Purpose:

Under the supervision of a pharmacist, the medication history technician (MHT) will obtain medication histories through interviews with the patient/family/care partner. The MHT will use technology to pull in external fill history from patients� pharmacies.

Essential Responsibilities:

  • Obtains and clarifies patient�s home medication list, including prescription, OTC, and herbal products. Any bottles brought in will be promptly returned to the patient�s family/care partner after all important information has been transcribed.

  • Responds to nurses, physicians, pharmacists, or other requests to manage, triage appropriately, and ensure follow-up promptly regarding med history information.

  • Obtains patient�s allergies & intolerances and communicates to appropriate caregiver for proper documentation into the electronic medical record (EMR), including symptoms, reactions, and severity.

  • Directs, triages, and communicates to appropriate caregiver all questions and inquiries from patient/family/care partner outside the scope of the MHT.

  • Identifies and participates in ongoing quality assurance and improvement processes to obtain an appropriate med history list.

  • Fulfills all compliance responsibilities related to the position.

  • Demonstrates regular, reliable and predictable attendance.

  • Performs other duties as required

Required:

  • High School Diploma or equivalent

  • Minimum Experience: 1 year

Desired:

  • Retail Pharmacy Technician experience.

  • Computer skills, ability to multi-task and work independently.

License, Registration, or Certification Requirements:

  • PTCB Certification within 6 months

  • License: Registered Pharmacy Technician with NY Board of Pharmacy within 6 months
