  • Communicate with clients and family members and answer clinical questions.

  • Track changes in client condition, communicate concerns to the appropriate team members and caregivers, and follow up as needed.

  • Provide orientations, ongoing education, competency evaluations, and in-field support to HHAs and PCAs.

Requirements

  • Unencumbered New York State RN license

  • Current CPR/BLS certification

  • Ability to travel for assessments and visits in Brooklyn and Manhattan, with occasional travel to Queens and the Bronx

  • Strong organizational skills

  • A detail-oriented approach to clinical assessments and documentation

  • Someone who can follow established procedures while using sound judgment and working independently

Benefits

  • Starting rate of $45/hour, with a rate increase after three months

  • Holiday pay

  • Sick pay

  • On-the-job training and support

  • Opportunity to build a schedule around other professional commitments

  • NYS Secure Choice Savings Program (Roth IRA)

  • A collaborative, close-knit team where your work MATTERS

Interested? If you’re an RN who values person-centered care and enjoys taking ownership of your work while being part of a supportive team, we’d love to hear from you.
