Geriatric Primary Care - Nurse Practitioner or Physician Assistant

We are seeking a part-time (or full-time) Nurse Practitioner or Physician Assistant for our outpatient primary care clinic at Seabury Active Life Plan Community in Bloomfield, CT to deliver compassionate geriatric primary care. You will carry your own primary care panel and do geriatric consults. We will provide comprehensive training to ensure our geriatric patients receive the best care. In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists. Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physicians. Requirements: Licensure: Unrestricted State of Connecticut Medical License (or eligible) Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA Experience: 2 of post-graduate experience as a Nurse Practitioners (APRN) or Physician Assistants (PA) in primary care and/or geriatric medicine Strong new grads with relevant nursing experience are welcome to apply! This is a salaried position. While compensation is presented as an hourly rate for consistency across our postings, this position is paid on a salary basis.
