Job Duties

We’re looking for a compassionate and organized Waiver Case Manager to join our team and support children and adults with disabilities. In this role, you’ll conduct MnCHOICES assessments to determine eligibility for long-term support services and develop person-centered plans that promote safety, independence, and quality of life.

You’ll provide ongoing case management for individuals eligible for Home and Community-Based Services (HCBS) Waivers, coordinating services, monitoring progress, and ensuring compliance with DHS guidelines. You’ll also collaborate with families, caregivers, and community professionals to build strong support networks.

Minimum Education Required

Minimum Qualifications: A bachelor's degree from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field. OR A bachelor's degree from an accredited four-year college or university with a major in any field and one year of experience as a social worker in a public or private social services agency. Bilingual (English/Spanish) highly valued.
