Job Duties

Health and Human Services of Faribault and Martin Counties is seekng a compassionate and skilled Family Preservation Case Manager to join our team. In this vital role, you will focus heavily on keeping children safe, families unified and working toward reunificiation when separated. You will build trusting relationships with families, helping them identify strengths, develop safety plans, and access resources that allow children to remain safely in their homes whenever possible.

Your work will include collaborating with law enforcement, schools, and legal professionals, preparing families for court involvement when necessary, and advocating for reunification when children have been placed out of the home. You’ll also provide on-call coverage, intake services and maintain accurate documentation in compliance with state and agency guidelines.

This position requires strong knowledge of child welfare laws, cultural competence, and a trauma-informed, family-centered approach. You’ll be expected to communicate effectively, participate in reflective supervision and ongoing training, and contribute to a collaborative team environment. Professionalism, ethical boundaries, and a commitment to continuous learning are essential.

You’ll report to the Family Preservation Supervisor and work closely with a multidisciplinary team.

Minimum Education Required

Minimum Qualifications: A bachelor's degree from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field. OR A bachelor's degree from an accredited four-year college or university with a major in any field and one year of experience as a social worker in a public or private social services agency. Bilingual (English/Spanish) highly valued.
