Registered Nurse

We are seeking a Registered Nurse (RN's & LPN's) to join our team. In this role, along with Admission / Assessments / Reassessments, you will primarily create and manage a care plan for individual patients. Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans. Administer medications and monitor patients for side effects or reactions. Document any changes in patient conditions and report on changes. Maintain accurate records of patient care and discuss observations with the supervisor. Currently licensed New York State Registered Nurse (RN). Previous experience as a Registered Nurse is preferred. Excellent interpersonal and client care skills. Valid drivers license. First aid/CPR certification required. Provides compassionate, reliable, and respectful care in the client's home. Follows the established care plan, agency policies, and applicable New York State requirements. Maintains a safe, clean, and supportive environment for the client. Works cooperatively with clients, families, supervisors, and other healthcare professionals. May work variable schedules, including weekends, holidays, evenings, or overnight assignments IF NEEDED!
