HomeWell Care Services – Chula Vista, CA Employment Type: Part-Time, W-2 Hours: 15–20 hours per week Pay: $27–$36 per hour, depending on credentials and experience Location: Chula Vista / South Bay San Diego and surrounding areas
About the Position
HomeWell Care Services provides personalized, non-medical in-home care that helps seniors and other adults remain safe, comfortable, and independent at home.
We are seeking an experienced and compassionate Part-Time Client Care Manager to oversee client-care quality, support caregivers, communicate with families, and ensure each client's Plan of Care continues to meet their needs.
This is an excellent opportunity for an experienced CNA or LVN ready to move into care coordination, client relations, caregiver support, and quality assurance.
This is primarily a care-management position, not a direct-care caregiving position.
Key Responsibilities

Conduct in-home client assessments and reassessments.

Develop, review, and update individualized Plans of Care.

Conduct supervisory and quality-assurance visits.

Follow up with clients and families after new starts of care.

Monitor client satisfaction and identify concerns before they escalate.

Recognize and report changes in clients' condition, safety, mobility, behavior, or care needs.

Assist with caregiver-client matching based on skills, personality, and client needs.

Work closely with the Scheduler/Care Coordinator regarding client and caregiver concerns.

Coach caregivers on Plans of Care, client expectations, professionalism, and performance.

Address client and family concerns promptly and professionally.

Support new starts of care and help ensure a smooth transition.

Maintain accurate and timely documentation of assessments, visits, incidents, and follow-ups.

Escalate significant client or caregiver concerns appropriately.

Maintain confidentiality and professional boundaries.
Preferred Qualifications
-
Current California CNA certification or LVN license preferred.
-
Minimum 2 years of senior-care experience, preferably in home care, senior living, hospice, home health, skilled nursing, or a related setting.

Experience with client assessments, care coordination, supervisory visits, caregiver supervision, or case management preferred.

Experience with dementia, Alzheimer's disease, chronic conditions, and mobility limitations.

Strong communication, documentation, organization, and problem-solving skills.

Confident communicating with clients, families, and caregivers.

Comfortable coaching caregivers and addressing performance concerns.

Valid California driver's license, reliable transportation, and ability to travel throughout the service area.
Ideal Candidate
We are looking for someone who is compassionate, confident, dependable, organized, and accountable.
The ideal candidate is comfortable visiting clients in their homes, assessing how services are going, communicating with families, coaching caregivers, recognizing changes in client needs, and following concerns through resolution.
Schedule & Compensation
Approximately 15–20 hours per week, primarily weekdays, with flexibility for assessments, supervisory visits, and new starts of care.
Pay: $27–$36 per hour, based on credentials and relevant experience.

Experienced CNA/senior-care professional: approximately $27–$30/hour

Experienced LVN with strong care-management experience: approximately $31–$36/hour
Mileage reimbursement is provided for authorized business travel in accordance with company policy.
