Full job description

Pharmacy1st Health is building the clinical infrastructure for rural America by transforming independent pharmacies into integrated care delivery hubs. Our model combines primary care, pharmacy services, and chronic disease management under one roof, creating a new, lower-cost site of care for underserved communities. We operate across multiple states and are rapidly expanding a network of rural clinics co-located with pharmacies. Our goal is to deliver scalable, high-quality care while improving access, outcomes, and cost efficiency. Description From day one, you will play a central role in shaping how care is delivered in a community where access truly matters. You will practice with autonomy, purpose, and support while helping launch a modern, community based care model that blends primary care, prevention, and technology enabled access. You will also help influence the growth of the care team by participating in the recruitment and selection of future clinical roles, ensuring we build a team aligned in values, quality, and commitment to the community. This is an opportunity for a clinician who wants to make a visible difference, build something lasting, and be known and trusted in the community they serve. Why This Role Is Special

  • Be a founding clinician with real ownership in how the clinic operates and grows

  • Practice at the top of your license with time to build meaningful patient relationships

  • Help redefine rural healthcare through integrated, team based care

  • Use telehealth and digital tools to expand access without sacrificing quality

  • Join a mission led organization that values clinical judgment and community trust

What You Will Do

  • Deliver comprehensive primary care with a strong focus on prevention and continuity

  • Serve as a clinical leader and trusted partner to patients and families

  • Collaborate with pharmacists, care coordinators, and virtual care partners

  • Help design workflows, care standards, and patient experience from the ground up

  • Contribute ideas and leadership as the clinic scales and evolves

Who You Are

  • A Family Nurse Practitioner with a Master’s or Doctoral degree in Nursing

  • Licensed in the job's respective state with national certification such as FNP-BC or FNP-C

  • Experienced in primary care, community health, or rural health environments

  • Confident using EHR systems and telehealth platforms

  • Entrepreneurial, adaptable, and excited by building something meaningful

  • Deeply committed to patient centered, relationship driven care

Compensation and Benefits Benefit offerings are for full-time employees and eligibility details will be finalized prior to hire. We are committed to building benefits and schedules that support both professional excellence and clinician wellbeing.

  • Relocation Bonus

  • Employer sponsored healthcare coverage designed to support your overall health, with plan details to be finalized

  • Paid time off intended to encourage rest, balance, and long term sustainability

  • Dedicated mental health and wellness days in addition to PTO

  • Paid holidays

  • Schedules intentionally designed to support strong patient care and a healthy life outside the clinic

  • Purpose driven work with real and lasting community impact

  • Founding role with long term growth and leadership opportunity
