Full job description
Join our innovative clinical team as a Part-Time / Per Diem Telehealth Physician. This role offers a unique opportunity to break away from traditional clinic walls and deliver high-impact, compassionate care to frail, elderly, and medically complex patients right where they live - partnering directly with skilled, cross-trained paramedics at the bedside.
The Role As a Telehealth Physician, you will harness advanced mobile diagnostics and therapeutics to evaluate and treat acute symptoms directly in patients' homes. You will design targeted treatment plans, identify readmission risks, and empower patients and families through practical health education - significantly curbing avoidable hospitalizations. Working alongside a responsive interdisciplinary team, you will pair tech agility with clear, compassionate communication in an adaptable, fast-paced environment. This position offers an exceptional opportunity for Hospitalists and Emergency Medicine Physicians seeking meaningful, flexible clinical work that actively reshapes acute healthcare delivery. Join us in bringing safe, quality care to our patients in their homes.
What You'll Do
• Deliver Advanced Virtual Care: Conduct comprehensive telehealth evaluations, diagnoses, and treatment plans for acute and complex patients from the comfort of a remote setting.
• Direct High-Acuity In-Home Interventions: Partner in real time with cross-trained field responders (paramedics) at the patient’s bedside to guide hands-on procedures - including bedside ultrasound (POCUS), IV therapy and medication administration, lab draws, point-of-care testing, respiratory support, foley catheter care, and more.
• Collaborate Across Clinical Teams: Serve as an expert clinical resource, consultant, and escalation point for our team of Nurse Practitioners and Physician Assistants.
• Coordinate Seamless Care Transitions: Facilitate integrated care coordination across external providers, specialists, and referring clinical teams through timely documentation and live clinical handoffs.
• Maintain Accurate Documentation: Complete timely, thorough visit documentation within the EHR to ensure continuity of care and clinical excellence.
Scheduling Commitment: Minimum of 4 days of availability per month, including at least 1 weekend day
Hours of Operation: 7 days a week, 9:00 AM – 9:00 PM EST Shift Length: 8- to 12-hour shifts
