Position Responsibilities

Provide comprehensive outpatient Family Medicine care to patients across the lifespan.

Manage acute illnesses, chronic conditions, preventive care, wellness visits, and routine follow-up.

Establish and maintain an ongoing patient panel.

Perform histories, physical examinations, assessments, diagnosis, and treatment planning.

Order and interpret diagnostic testing and coordinate referrals when appropriate.

Provide patient education, counseling, and shared decision-making.

Collaborate with physicians, APPs, specialists, nursing staff, and ancillary services.

Participate in quality improvement and other clinical initiatives.

Deliver care consistent with professional standards, organizational policies, and applicable regulations.
Qualifications

Graduate of an accredited Nurse Practitioner or Physician Assistant program.

Minimum of one year of post-education clinical experience required.

Experience in Family Medicine, primary care, or a closely related outpatient setting preferred.

Current board certification through an appropriate national certifying organization.

Licensed or ability to obtain license as an NP or PA in Minnesota.

Licensed or ability to obtain DEA licensure.
