Full job description

Description

This is an opportunity to practice as part of a collaborative, experienced healthcare team focused on proactive geriatric care. By visiting patients on-site each week, the Extended Care team identifies and addresses changing health needs early—helping prevent avoidable complications, hospitalizations, and disruptions in care.

You will work closely with nursing home staff, physicians, families, and other healthcare professionals to manage both acute concerns and complex chronic conditions. The role offers meaningful clinical relationships, professional autonomy, and the support of a shared practice model.

What you’ll do

  • Provide comprehensive primary and geriatric care to residents in skilled nursing facilities

  • Conduct health assessments, screenings, follow-up visits, and routine clinical evaluations

  • Diagnose and treat acute illnesses, chronic diseases, and changes in patient condition

  • Develop, implement, and adjust individualized treatment plans in collaboration with the nursing home physician and care team

  • Provide health promotion, disease prevention, education, and counseling to patients, families, and caregivers

  • Order, perform, supervise, and interpret appropriate diagnostic studies, within applicable scope and regulations

  • Prescribe pharmacologic and non-pharmacologic treatments

  • Coordinate care with nursing home staff, physicians, specialists, therapists, and other healthcare providers

  • Consult with, collaborate with, or refer to other providers when indicated by patient needs

  • Support thoughtful transitions of care and continuity across healthcare settings

  • Contribute to a culture of clinical excellence, teamwork, and compassionate service

Why join the Extended Care team?

  • Make a meaningful difference in the lives of older adults and their families

  • Build lasting patient relationships through consistent, on-site care

  • Practice proactively, with time to understand the whole patient—not just the immediate problem

  • Work alongside a collaborative team that values communication and shared clinical decision-making

  • Enjoy a role that combines autonomy with strong physician and organizational support

  • Be part of Mankato Clinic, a respected physician-led organization serving the region

Qualifications

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program

  • Current and unrestricted Minnesota license, or eligibility to obtain and maintain Minnesota licensure

  • Board certification through AANP, ANCC, NCCPA, or the applicable national certifying organization

  • Ability to obtain and maintain Minnesota DEA certification

  • Certified or desire to obtain Wound Certification preferred

  • At least two years of professional clinical experience in family practice, internal medicine, geriatrics, or a related setting

  • Geriatric experience, training, or a strong interest in caring for older adults preferred

  • Valid driver’s license and minimum state-required automobile liability insurance

  • Ability to travel locally to area skilled nursing facilities

Compensation

$96,824–$145,080 annually, based on experience, plus bonus opportunity.

Benefits
