We are seeking a full-time Nurse Practitioner or Physician Assistant for our REACH (Resource for Elderly and Care at Home) Program, delivering compassionate primary care in the comfort of geriatric patients’ homes and assisted living facilities. Role: Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA) Location: Home-based geriatric care based in the greater Hartford, CT area Setting: Geriatric Medicine/Primary Care in the home setting, working within a multi-disciplinary care team Schedule: Full-time, weekdays only! In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists. Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physicians. Requirements: Licensure: Unrestricted State of Connecticut Medical License (or eligible) Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA Experience: At least two years of post-graduate experience as a Nurse Practitioners (APRN) or Physician Assistants (PA). Primary Care/Geriatric Medicine experience highly preferred. New grads with relevant nursing experience are welcome to apply!
