Licensed Practical Nurse (LPN) – Full Time!

KeystoneCare is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide private duty nursing care for a patient with ALS in their home. This role is ideal for a nurse who is committed to delivering high-quality, patient-centered care while supporting both the patient and their family.

  • Provide skilled nursing care in accordance with physician orders, patient care plans, and agency policies
  • Administer medications and treatments as prescribed
  • Perform ongoing patient assessments and monitor for changes in condition
  • Ensure patient safety, comfort, and overall well-being throughout the shift
  • Communicate effectively with patients, families, and interdisciplinary care teams
  • Educate patients and families on care plans, equipment use, and safety measures
  • Maintain accurate and timely documentation in compliance with agency standards

Requirements Job Requirements:

  • Graduate of an accredited LPN program
  • Current LPN license in Pennsylvania
  • Minimum of one year experience in home healthcare or a similar clinical setting
  • Familiarity with Feeding Tubes, Assisted breathing Devices (Ventilators) and Hoyer Lift
  • CPR certification
  • Strong communication and interpersonal skills
  • Self-motivated with the ability to work independently
  • Reliable transportation and valid Driver's License
