Full job description
Apollo Home Healthcare is seeking a Speech–Language Pathologist (SLP) to provide in-home speech therapy visits across Contra Costa County, Alameda County, and the Tri-Valley area. This is a W-2 per diem role ideal for SLPs looking for schedule flexibility, autonomy, and meaningful patient relationships. You’ll provide high-quality, one-on-one care to patients in their homes while collaborating with a supportive interdisciplinary team.

What You’ll Do
As a Home Health SLP, you will:

Perform speech-language and swallowing evaluations in patients’ homes

Develop and implement individualized Plans of Care
-
Provide skilled speech therapy interventions
-
Recommend communication devices and adaptive equipment as needed
-
Assess home environments for safety and functional communication needs

Collaborate with physicians, nurses, PT/OT, and care teams
-
Participate in case conferences as appropriate

Complete timely, compliant clinical documentation

Educate patients and caregivers on exercises and compensatory strategies

Supervise home health aides when applicable to the plan of care
Requirements
-
Active California Speech–Language Pathologist license
-
Master’s degree in Speech-Language Pathology
-
Current CPR certification
-
Home health experience preferred (but not required)

Strong documentation and communication skills
-
Ability to work independently in the field
-
Reliable transportation and valid driver’s license
