About the Role: ComForCare Home Health Care (North San Diego) is looking for a compassionate and skilled LVN to join our dedicated home health team in San Diego, CA. This is a rewarding opportunity to make a real difference in patients' lives by delivering personalized, one-on-one care right in the comfort of their homes. Responsibilities:

  • Provide skilled nursing care to home health patients in accordance with physician-approved care plans

  • Administer medications, wound care, injections, and other LVN-scope treatments

  • Monitor and document patient health status, vital signs, and progress toward care goals

  • Communicate effectively with RNs, physicians, and interdisciplinary care team members

  • Educate patients and family members on disease management, medications, and home safety

  • Maintain accurate and timely clinical documentation in compliance with home health regulations

  • Build trusting relationships with patients to support continuity of care and positive outcomes Requirements:

  • Valid California LVN license in good standing

  • Prior home health, skilled nursing, or clinical experience preferred

  • Current CPR/BLS certification

  • Valid driver's license, reliable transportation, and active auto insurance

  • Strong clinical assessment and critical thinking skills

  • Excellent communication, documentation, and time management abilities
