About the Role: ComForCare Home Health Care (North San Diego) is looking for a compassionate and skilled LVN to join our dedicated home health team in San Diego, CA. Responsibilities:

Provide skilled nursing care to home health patients in accordance with physician-approved care plans

Administer medications, wound care, injections, and other LVN-scope treatments
-
Monitor and document patient health status, vital signs, and progress toward care goals
-
Communicate effectively with RNs, physicians, and interdisciplinary care team members
-
Educate patients and family members on disease management, medications, and home safety
-
Maintain accurate and timely clinical documentation in compliance with home health regulations
-
Build trusting relationships with patients to support continuity of care and positive outcomes Requirements:

Valid California LVN license in good standing
-
Prior home health, skilled nursing, or clinical experience preferred
-
Current CPR/BLS certification
-
Valid driver's license, reliable transportation, and active auto insurance
-
Strong clinical assessment and critical thinking skills

Excellent communication, documentation, and time management abilities
