Subacute Coordinator Salaried: $83,200 - $87,360 (depending on experience) Equivalent Hourly: $40- $42 Sign-On Bonus: $10,000!! Shift: Full-Time (40 hours/week) Schedule: Monday-Friday, 12 pm - 8 pm; rotating on-call schedule
Susquehanna Nursing & Rehabilitation Center is looking to add an Subacute Coordinator to its nursing leadership team. In this role, you'll oversee the daily operations of an assigned nursing unit, ensuring high-quality resident care, staff supervision, regulatory compliance, and exceptional customer service. RESPONSIBILITIES:

Direct, coordinate, and supervise the activities of nursing staff on an assigned unit

Assume responsibility and accountability for the overall management of resident care within the unit

Monitor resident conditions and ensure care plans are developed, maintained, and updated as needed

Collaborate with physicians, interdisciplinary team members, residents, and families to support quality care outcomes
-
Communicate staff concerns and operational issues to appropriate department leaders
-
Ensure compliance with facility policies, state and federal regulations, and nursing standards of practice

Support staff development, coaching, performance management, and team engagement
-
Assist with admissions, discharges, family communication, and resident care coordination

Promote a safe, positive, and resident-centered environment

Perform additional duties as assigned
REQUIREMENTS:

Degree from an accredited School of Nursing

Current and valid New York State Registered Nurse (RN) License required

Minimum of one year of experience in nursing service administration required

Additional experience in rehabilitative or geriatric nursing preferred

Strong knowledge of state, federal, and local long-term care regulations

Excellent leadership, communication, and organizational skills

Strong customer service skills and a compassionate approach to resident care

Must be flexible, dependable, professional, and able to manage multiple priorities
