Licensed Practical Nurse (LPN) - All Shifts Available
Join our team at The Center at Blue Hills, a dynamic and supportive healthcare environment!
As a Licensed Practical Nurse (LPN) at The Center at Blue Hills, you will play a vital role in delivering high-quality patient care to our residents. We are seeking a compassionate and skilled LPN to join our team on all shifts, including days, evenings, and nights.
Responsibilities:

Provide direct patient care under the supervision of a registered nurse, including administering medications, performing vital signs, and assisting with activities of daily living

Monitor and report changes in patients' conditions to registered nurses and other healthcare team members

Develop and implement individualized care plans to meet the unique needs of each patient
-
Collaborate with healthcare team members to ensure effective communication and coordination of patient care
-
Maintain accurate and detailed records of patient care and treatment
-
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

Current LPN license in the state of Massachusetts

High school diploma or equivalent required

Previous experience in a long-term care setting preferred but not required
