We are seeking a compassionate and skilled LPN (Licensed Practical Nurse) to join our dedicated healthcare team at The Gardens at Cedarwood in Franklin, Massachusetts, United States. Responsibilities:

Provide high-quality nursing care to patients in accordance with established policies, procedures, and standards of care

Conduct initial and ongoing assessments of patient needs and develop individualized care plans

Administer medications, treatments, and therapies as prescribed by physicians and other healthcare providers

Collaborate with other healthcare professionals to ensure comprehensive care and effective communication

Maintain accurate and up-to-date patient records and reports

Participate in quality improvement initiatives to improve patient outcomes and satisfaction
Requirements:

Current LPN licensure in the state of Massachusetts

Ability to work effectively as part of a team and communicate with patients, families, and other healthcare professionals

Strong assessment, planning, and problem-solving skills

Ability to prioritize tasks and manage time effectively

Compassion, empathy, and a commitment to delivering high-quality patient care
