PRN Registered Nurse (RN) – Home Care & Client Education

ComForCare Home Care – North Austin Position Type: PRN / As Needed

ComForCare Home Care – North Austin is seeking an experienced and compassionate PRN Registered Nurse (RN) with a background in home care and geriatric care.

This is a flexible, as-needed position focused on client assessments, start of care, RN delegation, education, care coordination, and clinical support. This is not a traditional bedside or Private Duty Nursing (PDN) position.

Key Responsibilities

  • Conduct initial client assessments and start-of-care visits as needed.

  • Develop, review, and update individualized care plans.

  • Perform RN delegation, when appropriate and permitted under Texas regulations, including caregiver instruction, competency validation, supervision, and documentation.

  • Assess whether client-specific tasks are appropriate for delegation and provide required follow-up and oversight.

  • Educate clients, families, and caregivers regarding care needs, safety, fall prevention, mobility, nutrition, medication awareness, and aging safely at home.

  • Identify changes in client condition or needs and communicate appropriate recommendations and next steps.

  • Conduct reassessments, supervisory visits, and other nursing visits as needed.

  • Coordinate with families, caregivers, physicians, home health, hospice, therapy providers, and other healthcare professionals as appropriate.

• Accompany clients to medical appointments when requested, helping facilitate communication and understanding of follow-up instructions.

  • Support clients and families during changes or transitions in care.

  • Maintain accurate and timely documentation.

  • Serve as a clinical resource and educator for caregivers and the home care team.

Qualifications

  • Current, unrestricted Texas RN license.

• Home care or home health experience required.

  • Experience working with geriatric/senior populations required.

  • Knowledge and experience with Texas RN delegation strongly preferred.

  • Experience with client assessments, care planning, caregiver education, and family education.

  • Strong communication, assessment, documentation, and organizational skills.

  • Comfortable working independently in clients' homes.

  • Valid driver's license and reliable transportation.

Schedule

This is a PRN/as-needed position. Hours will vary based on new client starts, assessments, delegation needs, supervisory visits, client appointments, and other clinical needs. No minimum or guaranteed number of hours is implied.
