LOOKING TO HIRE RN/LPNSPay: $45.00 - $50.00 per hourJob description:Surrogate Family Care LLC, a Licensed Home Care Services Agency (LHCSA), is hiring RNs and LPNs to provide one-on-one skilled nursing care for medically fragile pediatric, adult, and geriatric Medicaid members in their homes throughout New York. New grads welcome!Why Join Us * Competitive pay weekly direct deposit * Health benefits * Flexible scheduling: Full-Time, Part-Time, Per Diem * 8 / 10 / 12-hour shifts available * Paid training & orientation * Quick and smooth hiring processWhat Youll Do * Medication administration (oral, nebulizer, IV, enteral) * G-tube, J-tube & NG-tube feedings monitor formula, rate, volume & schedule * Tracheostomy care, suctioning (nasal, oral, tracheal) & stoma care * Ventilator & BiPAP/CPAP monitoring per physician-ordered settings * Oxygen therapy & pulse oximetry monitoring with parameter-based intervention * Chest physiotherapy, vest therapy & cough-assist treatments * Glucose monitoring, seizure management & bowel regimen care * Wound care, dressing changes, ostomy care & catheter management * Patient assessments & detailed clinical documentation each shift * Family & caregiver education on skilled tasks and emergency procedures * Coordination with physicians, nurse practitioners & care teamsRequirements * Active NYS RN or LPN license (current NYSED registration) * Current BLS / CPR certification * Eligible to work in the United States * Reliable transportation to patient locationsPreferred * Home care or private duty nursing experience * Experience with vent-dependent, trach-dependent, or medically fragile pediatric patients * Familiarity with NYS Medicaid PDN documentation & prior approval processes * IV infusion certification a plus (we offer certification to willing RNs) * Bilingual a plusGood to Know * PDN is for skilled nursing tasks only (not household chores, bathing, or feeding) * Nurses may not exceed 16 hours of service in a 24-hour period * Nurses may accompany members in the community but may not drive the memberApply Today 5168062223 Ext 118Quick and smooth hiring process!Job Types: Full-time, Part-time, Per diemBenefits: * Paid time offWork Location: In person
