Physical Therapist Assistant (PTA)Rate: $35/hourShift: Per-DiemSchedule: Days

Taconic Rehabilitation & Nursing at Ulster is looking to add a Physical Therapist Assistant (PTA) to its rehabilitation team. In this role, you'll work under the supervision of a Physical Therapist to provide therapy services that help residents improve mobility, strength, balance, and overall function. RESPONSIBILITIES:

  • Provide skilled physical therapy treatments under the direction of a Physical Therapist

  • Assist with implementing individualized treatment plans designed to improve mobility, strength, balance, and functional independence

  • Monitor resident progress and communicate treatment updates to the supervising Physical Therapist

  • Complete required documentation, treatment records, and care plan updates accurately and timely

  • Educate residents, families, and staff on therapy techniques, exercises, mobility, and adaptive equipment

  • Assist with discharge planning, home exercise programs, and equipment recommendations

  • Collaborate with physicians, nursing staff, and interdisciplinary team members to support resident outcomes

  • Participate in staff training, in-services, and quality improvement initiatives

  • Maintain therapy equipment, supplies, and treatment areas

  • Perform additional duties as assigned

REQUIREMENTS:

  • Associate Degree from an accredited Physical Therapist Assistant program approved by the American Physical Therapy Association

  • Valid New York State Physical Therapist Assistant License required

  • Newly licensed Physical Therapist Assistants encouraged to apply

  • Skilled nursing facility experience preferred

  • Strong communication, documentation, and organizational skills

  • Ability to work collaboratively within an interdisciplinary healthcare team

  • Commitment to providing compassionate, resident-centered care
