Job Description:

Baptist Health Rehab Services provides a full range of physical, occupational, and speech therapy services for all ages. Our therapists and assistants are trained in treating neurological, orthopedic, vestibular, lymphedema, pediatrics, industrial medicine, swallowing, and speech and language disorders.

The staff physical therapist shall evaluate and treat patients, develop a treatment plan, and set goals for patients. Patient and family education is included in the treatment plan and goals. The PT will complete documentation according to applicable national and state regulations as well as hospital and departmental policies. The therapist will assist with the education of affiliating students, department staff, hospital staff, and the community at large. The therapist will appropriately delegate to and supervise physical therapist assistants and technicians. The therapist will participate in departmental operational plan. The therapist must intervene appropriately and meet the needs of patients with impaired mental abilities, vision, hearing, or mobility.

Job Description:

Completes appropriate chart / medical record review.

Follows plan of treatment appropriately, notifies referring provider of change in status and need for modifications to treatment plan or goals.

Plans treatment appropriately, including input from patient.

Receives and implements therapy orders.

Completes evaluation form, treatment notes, progress reports, and D/C summaries in a timely manner, and appropriately submits to referring provider.

Supervises/delegates/directs activities of students and Rehab Services Technicians.

Requirements:

Bachelors Degree in Physical Therapy

Current license by the Ky Board of Physical Therapy

BLS within 30 days from date of hire
