Responsibilities

✔️ Conduct client assessments and reassessments ✔️ Develop and oversee individualized care plans ✔️ Supervise and support caregivers ✔️ Monitor clients’ health status and changing needs ✔️ Communicate with families and healthcare professionals ✔️ Help seniors remain safe, independent, and comfortable at home

Qualifications
