Occupational Therapist (OTR/L) – CAPABLE Program & Adult Day Health Care (ADHC)

Location: Community-Based/Home Visits (Manhattan & Bronx)

Department: Adult Day Health Care (ADHC) / CAPABLE Program

Reports To: Project Coordinator

Schedule: Full-Time or Part-Time

Salary: $111,305.22 annually (Full-Time)

Sign-On Bonus

We are pleased to offer a $3,000 sign-on bonus for full-time hires and $1,500 for part-time hires, payable as follows:

  • 50% after six (6) months of employment

  • 50% after one (1) year of employment

About the Opportunity

Join an innovative team dedicated to helping older adults live safely and independently in their homes. We are seeking a compassionate and experienced Occupational Therapist (OTR/L) to support our evidence-based CAPABLE (Community Aging in Place, Advancing Better Living for Elders) Program within Adult Day Health Care services.

In this community-based role, you will work closely with a Registered Nurse and an interdisciplinary care team to conduct home-based assessments, identify barriers to daily functioning, and develop individualized interventions that promote independence, safety, and quality of life. Through collaborative care planning and home modification recommendations, you will play a critical role in helping participants age in place while reducing the risk of hospitalization and long-term institutional care.

This position is ideal for a clinician who is passionate about serving older adults and committed to delivering person-centered care in the community.

Key Responsibilities

  • Conduct comprehensive participant screenings and assessments for the CAPABLE Program.

  • Perform in-home evaluations to assess functional abilities, environmental barriers, and home safety concerns.

  • Evaluate physical, cognitive, emotional, and sensory functioning.

  • Assess participants' ability to perform Activities of Daily Living (ADLs) and develop individualized intervention plans.

  • Identify risks that impact safety and independence and recommend appropriate home modifications and adaptive solutions.

  • Prepare work orders and coordinate with vendors regarding approved home modifications.

  • Educate participants, families, and caregivers on adaptive equipment, home safety strategies, and therapeutic recommendations.

  • Maintain accurate and timely clinical documentation, including assessments, progress notes, care plans, and reports.

  • Ensure compliance with CAPABLE program standards, regulatory requirements, and organizational policies.

  • Collaborate with interdisciplinary team members to support participant goals and quality improvement initiatives.

  • Coordinate and manage scheduling of participant home visits and assessments.

  • Prepare program reports, outcome measures, and statistical data as required.

  • Provide rehabilitation therapy services within the Adult Day Health Care Program, as assigned.

  • Participate in professional development, continuing education, and CAPABLE-specific training.

  • Maintain ongoing communication with the Project Coordinator regarding participant progress and program operations.

CAPABLE Program Training

The selected candidate will complete a 10-hour virtual CAPABLE training program focused on participant assessment, care planning, intervention implementation, and outcome evaluation.

Qualifications Required Qualifications

  • Master's Degree in Occupational Therapy from an accredited program.

  • Current, active, and unrestricted New York State Occupational Therapist license.

  • Minimum of three (3) years of home care experience in a Certified Home Health Agency (CHHA), Long-Term Home Health Care Program (LTHHC), or similar community-based setting.

  • Experience conducting home safety assessments and recommending environmental modifications.

  • Strong knowledge of occupational therapy principles, treatment approaches, adaptive equipment, and safety practices.

  • Proficiency in electronic health records (EHRs) and standard computer applications.

  • Valid driver's license and ability to travel throughout assigned service areas.

  • Excellent organizational, documentation, and time-management skills.

  • Strong communication, interpersonal, and relationship-building abilities.

Preferred Qualifications

  • Bilingual language skills.

  • Experience working with older adults in community-based, geriatric, or aging-services settings.

  • Knowledge of aging-in-place programs and evidence-based interventions that support independence and reduce hospitalizations.
