Corewell Health Retail Pharmacy at Lakeland Hospital in St. Joseph is an open-door outpatient pharmacy located at St. Joseph Hospital. While this pharmacy provides traditional outpatient pharmacy services for the community, its main purpose is to operate a Discharge Medication Program to collaboratively work across professional disciplines to ensure patients discharging from the hospital leave with their new medications in hand. This is a excellent transition of care opportunity in the outpatient pharmacy setting.

Corewell Health Grand Rapids Hospitals Retail Pharmacy is a closed-door, outpatient pharmacy located at Butterworth Hospital in Grand Rapids, Michigan. This pharmacy operates under a “Specialty-Plus” Model, dispensing both Specialty and Mon-Specialty Medications pertinent to specific disease states that often require a higher level of care. This frequently involves a multidisciplinary, collaborative approach to ensure proper medication coordination and administration. Patient populations currently served include Hemophilia and Cystic Fibrosis.This is a wonderful opportunity to join an engaging, growing pharmacy space as we expand to provide superior, comprehensive care to a larger patient population.

Scope of Work
Provide daily patient focused clinical pharmacy services to inpatients and outpatients of all age groups of Lakeland Regional Health System to include, but not limited to, medication dosing, chart review, patient education, floor rounds and physician intervention. Participate on committees and give special reports. Rotate through the distributive pharmacy as scheduled.

Directly perform order entry in the pharmacy; Provide all of the functions as a staff pharmacist; Perform other duties as assigned.

Provide patient and community education; Participate in the staff development education program; Provide clinical training to Pharm. D. students as assigned.

Perform accurate and timely input of interventions and ADR’s.

Qualifications

Required Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy

2 years of relevant experience

American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice Preferred

LIC-Clinical Pharmacist - STATE_MI State of Michigan

CRT-Basic Life Support (BLS) - AHA American Heart Association
