Graduate Practical Nurse (GPN)Rate: $32.48/hourShift: Full-Time, Part-TimeSchedule: All Shifts!

Brookhaven Health Care Facility is looking to add a Graduate Practical Nurse (GPN) to its nursing team. In this role, you'll work under the supervision of a Registered Nurse to provide compassionate nursing care and support to residents while gaining valuable hands-on experience in a skilled nursing environment. RESPONSIBILITIES:

  • Provide nursing care under the supervision of a Registered Nurse, including medication administration and treatments

  • Assist with developing, implementing, and updating resident care plans

  • Monitor resident conditions and promptly report changes to the supervising nurse

  • Provide personal care assistance, including hygiene, mobility, and feeding support as needed

  • Accurately document resident care, treatments, and observations

  • Maintain a safe, comfortable, and resident-centered environment

  • Collaborate with nursing staff and interdisciplinary team members to support quality resident care

  • Follow facility policies, healthcare regulations, infection control practices, and safety procedures

  • Participate in team meetings and ongoing education opportunities

  • Perform additional duties as assigned

REQUIREMENTS:

  • High School Diploma or equivalent preferred

  • Graduate Practical Nurse eligible for licensure in New York State

  • Strong written and verbal communication skills

  • Ability to follow written and verbal instructions

  • Compassionate, resident-focused approach to care

  • Ability to work effectively with residents, families, physicians, and team members

  • Strong organizational and documentation skills

  • Ability to perform frequent physical activity and assist with resident care needs
