Job Description:

The PATH (Partnership, Advocacy, Transformation, Health) Care Coordinator (PCC) manages the service coordination needs of individuals enrolled in Tier Two of CHCI’s PATH Program. The goal of this role is to provide comprehensive care coordination and navigation through the complex healthcare and social services systems with a focus on elimination of barriers that may interfere with achievement of healthcare and social goals. The PCC will develop individualized care plans for clients with substance use disorders that prioritize the goals most important to the individual patient and will facilitate steps and referrals that are most likely to assist them in achievement of those goals. PCCs will assess needs, link patients to medical, behavioral health, and social resources, and monitor recovery progress. Core duties include patient intake, coordinating treatment, and maintaining documentation.

ROLE AND RESPONSIBILITIES

  • Develop and implement individualized service plans with clients, linking them to medical, housing, mental health, and legal support systems.

  • Conduct initial evaluations and ongoing screenings to determine client needs and treatment readiness.

  • Track client progress, adjust care plans as needed, and advocate for clients with healthcare professionals and state agencies.

  • Maintain accurate, timely, and confidential patient charts in compliance with HIPAA and facility regulations.

  • Conduct internal and external outreach to patients and community partners to raise awareness about the program and services available at CHCI, specifically treatment for substance use.

  • Actively recruit patients for participation in the PATH Substance Health Program and ensure that all sites have appropriate consent forms and disclosures for patient participation in program and research.

  • Work with Project ECHO providers on site to ensure that all patients enrolled in PATH SUH services are entered into CHCI database and appropriately flagged in CHCI EHR.

  • Ensure the detail, accuracy and timeliness of data entry and reporting.

  • Develop and implement retention strategies for patients in the program.

  • Work with the PATH SUH Program Director to ensure the progress of all outcome measures and performance improvement projects with relationship to the grant requirements.

  • Participate in community events, which will promote PATH SUH services and raise awareness about all services at CHCI.

  • Work with the PATH Project Director and Evaluator on specific evaluation goals.

  • Actively participate in all CKP, Interdisciplinary and Quality Improvement Meetings on a regular basis.

  • Identify and participate in trainings as appropriate for the professional development and accomplishment of all program goals.

  • Coordinate specialty appointments and provide additional support to address and reduce barriers to care for patients enrolled in the PATH Program.

QUALIFICATIONS

Bachelors in related field

Experience in outreach and care coordination

Experience in substance use field

Ability to communicate effectively with hard to reach populations
