Lake Sunapee VNA and Hospice is seeking an experienced Nurse Practitioner (NP) to join our Hospice program in a per diem capacity to provide direct patient care visits for routine provider visits, symptom management and Hospice Face to Face. On-call rotation required.

You Bring:

Experience in hospice and/or end of life care, and sympathetic management

Strong communication with demonstrated ability to collaborate with an interdisciplinary team

Comfortability with educating and guiding patients, families, facilities and community in end-of-life discussions, and decision-making

Qualifications:
-
Current NH Nurse Practitioner (NP) license
-
Minimum 2 years of health care experience required
-
Hospice, Palliative or end-of-life care experience is preferred
Additional requirements include reliable transportation, valid driver’s license, automobile insurance, current physical exam, TB testing, and required vaccinations.
