We're seeking a relationship builder with a genuine desire to help families. This role is especially well suited to an MSW, LSW, LPN, or other healthcare/community-services professional ready to take their advocacy and care coordination experience in a new direction.

What You'll Do

• Represent us at senior expos, health fairs, and community events

• Build referral relationships with senior centers, healthcare providers, discharge planners, social workers, nonprofits, and faith communities

• Respond promptly to family inquiries, listen to their needs, and help them understand their options

• Follow up consistently and track outreach toward measurable growth goals

• Partner with our care team for a smooth transition from first conversation to start of care

What We're Looking For

• 2+ years in sales, business development, social work, healthcare, community outreach, or a related field

• MSW, LSW, LPN, or experience with older adults, discharge planning, or care coordination is a strong plus

• Excellent listening, communication, presentation, and follow-up skills

• Confidence in starting conversations and managing a local territory independently

• Proficiency with CRM systems, Apple products, and Microsoft Office

• Valid driver's license, reliable transportation, and willingness to travel locally

• Sales experience is valuable, but we also welcome people who have built trust and connected others with services in other settings.
