Job Description

Licensed Practical Nurse (LPN)

Join Our Healthcare Team and Make a Difference in Patient Care

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our clinical team. In this role, you will provide high-quality, patient-centered care while working closely with physicians, nurses, and other healthcare professionals to support positive clinical outcomes.

We are looking for an LPN who is professional, dependable, organized, and committed to creating a positive experience for every patient.

What You'll Do

  • Provide safe, compassionate, and quality nursing care to patients in accordance with established policies, procedures, and standards of practice.

  • Work collaboratively with physicians, registered nurses, and other members of the healthcare team.

  • Monitor and assess patients and communicate changes in condition to the appropriate healthcare provider.

  • Maintain accurate and timely patient documentation in the electronic medical record.

  • Protect patient privacy and maintain strict confidentiality of patient information.

  • Establish professional and respectful relationships with patients, families, and coworkers.

  • Promote patient dignity, safety, comfort, and well-being.

  • Provide care appropriate to the patient's age, developmental needs, and clinical condition.

  • Participate in continuing education, in-service programs, competency assessments, and other professional development activities.

  • Contribute to a positive team environment through effective communication, professionalism, and role modeling.

  • Perform other duties as assigned.

What We're Looking For

Required:

  • Graduate of an accredited Practical Nursing program.

  • Current New York State LPN license.

  • Basic Life Support (BLS) certification through RQI required within 90 days of hire.

  • Strong interpersonal, communication, organizational, and customer service skills.

  • Ability to manage multiple priorities and meet deadlines.

  • Ability to handle confidential information with professionalism and discretion.

  • Basic computer literacy.

Preferred:

  • Previous physician office or outpatient healthcare experience.

  • Experience using electronic medical records (EMR).

  • Experience with Microsoft Office products.
