Physical Therapist (PT) – Certified Home Health Agency (CHHA)

Employment Status: Part-Time

Incentive Bonus: Eligible candidates will receive a $3,000 sign-on bonus, prorated for part-time employees.

Position Summary

The Certified Home Health Agency (CHHA) is seeking a dedicated and compassionate Physical Therapist to join our interdisciplinary home care team. This role is responsible for providing skilled physical therapy services to patients in the community and coordinating care with physicians, nurses, therapists, social workers, and other healthcare professionals.

The Physical Therapist will assess patients' functional abilities, develop individualized treatment plans, and implement therapeutic interventions designed to maximize independence, mobility, and overall quality of life.

Essential Duties and Responsibilities

  • Collaborate with physicians, patients, family members, nurses, social workers, and other members of the healthcare team to develop and implement individualized treatment plans.

  • Perform comprehensive physical therapy evaluations and ongoing patient assessments.

  • Develop, implement, and modify treatment programs based on patients' needs, goals, and progress.

  • Educate patients and caregivers on therapeutic exercises, mobility techniques, safety practices, and strategies to promote independence.

  • Monitor, document, and report patient progress in accordance with agency standards and regulatory requirements.

  • Observe and report changes in patient condition to physicians and appropriate members of the care team.

  • Complete OASIS assessments accurately and within established timeframes in accordance with regulatory guidelines.

  • Maintain timely and accurate clinical documentation, including evaluations, progress notes, reassessments, and discharge summaries.

  • Supervise Physical Therapist Assistants and support personnel in accordance with New York State regulations and agency policies.

  • Coordinate care with other disciplines to ensure continuity and effectiveness of patient treatment plans.

  • Participate in interdisciplinary team meetings, case conferences, and care planning activities.

  • Initiate and participate in case conferences for patients requiring complex or multidisciplinary interventions.

  • Perform additional duties as assigned.

Professional Skills and Competencies

The successful candidate will demonstrate the ability to:

  • Conduct comprehensive physical therapy evaluations and assessments.

  • Develop individualized treatment plans for a wide range of diagnoses and functional limitations.

  • Establish measurable treatment goals and expected outcomes.

  • Modify treatment plans based on patient response and progress toward goals.

  • Apply sound clinical judgment and evidence-based practice in patient care.

  • Communicate effectively, both verbally and in writing.

  • Provide leadership, guidance, and clinical instruction to support staff and team members.

  • Work independently while maintaining effective collaboration with the interdisciplinary healthcare team.

Qualifications Required

  • Current, active, and unrestricted New York State license as a Physical Therapist.

  • Graduation from an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA) or another recognized accrediting body.

  • Master's Degree or Doctorate Degree in Physical Therapy.

  • Strong clinical assessment, documentation, and patient care skills.

  • Ability to travel to patient homes within the assigned service area.

Preferred

  • Minimum of one (1) year of Physical Therapy experience.

  • Previous home health or community-based healthcare experience.

  • Experience working with adult and geriatric populations.

  • Knowledge of OASIS documentation requirements.
