Care Manager Assistant Boulder Community Health, Boulder, CO 80303 $19.29-$26.90 an hour- Half-Time Days, 16 hours per week Boulder Community Health is seeking a Care Manager Assistant to provide comprehensive administrative, clerical, and coordination support to the inpatient behavioral health unit. This role facilitates efficient patient care delivery by coordinating communication, managing documentation, supporting discharge planning and post-acute services, and ensuring compliance with regulatory and organizational standards. Benefits: •Health insurance, including a FREE employee only option •Dental and Vision insurance •BCH paid Life Insurance; Spouse and Dependent Life Insurance plans •Short-term and Long-term disability coverage •Health and Dependent Care Flexible Spending Accounts •Retirement plan with BCH matching contributions, and discretionary lump sum contribution •Paid Time Off •Education assistance program •Voluntary Wellness programs, to include biometrics, wellness team challenges, and much more •Staff Support Initiatives such as Sound Baths, Meditation, Massages, and Reiki •Free one-on-one retirement planning sessions •Employee Assistance Program offering 8 free, confidential counseling sessions for you and your family Qualifications •High school diploma or equivalent preferred •Associate degree preferred. •Previous health care, hospital, or medical office experience preferred. •Experience with Medical terminology care coordination processes preferred Responsibilities •Provide clerical, administrative, and secretarial support to unit staff and care management. •Manage phone calls, messages, and communication, serving as the primary point of contact for case management. •Working knowledge of Scoop and how to locate department forms, policies, procedures, phone numbers and iPrivileges. •Ability to function in Epic and Faxcom. •Serves as a resource to physicians, therapists, and other staff needing assistance. •Collaborate with interdisciplinary teams to support patient care coordination. •Under direction of the therapists and Clinical Supervisor, assists with scheduling follow up appointments for patients and coordinating post-acute services. •Facilitate family&/or caregiver communication, meetings, and scheduling •Support discharge planning by coordinating communication with external agencies and providers. •Coordinates exchange of PHI between the inpatient unit and other facilities or centers. •Coordinates patient legal documents and assures they are in the patient medical record. •Locates and provides resources to patients based on their specific needs, with the assistance of the therapists. •Arranges transportation for patient transfers and discharges. •Assumes responsibility for and manages specialty supply ordering for the unit. •Manages usage of the Anchor Point Mental Health Endowment in conjunction with the Director of Nursing. •Review DT efficiency monthly.
