Right at Home West Palm Beach & Vero Beach Connecting clients, caregivers, families, and Right at Home through trust, communication, and exceptional care. It begins with trust. It begins with relationships. It begins with having someone who listens, follows through, and makes sure the right people are connected at the right time. That’s where you come in. We’re looking for a Care Manager who understands that great home care is about much more than scheduling caregivers and completing assessments. It’s about creating a strong connection between our clients, their families, our caregivers, and the Right at Home team. The Care Manager is a trusted liaison between the people we serve and the agency that serves them—someone who can listen to a client’s concerns, recognize opportunities to improve care, support our caregivers, and make sure important information doesn’t fall through the cracks. You’ll be involved from the beginning of the client relationship, conducting initial assessments and helping families understand how Right at Home can support them. You’ll stay connected as care evolves, monitor important reports and outcomes, and help ensure our clients receive the quality, consistency, and responsiveness they deserve. You’ll also play an important role in developing and supporting our caregivers—from conducting new-hire orientations to helping them understand what it means to represent Right at Home in a client’s home. What You’ll Do Your responsibilities will include: Serve as a primary liaison between clients, families, caregivers, and the Right at Home team, ensuring concerns, questions, and important information are communicated and addressed promptly. Conduct initial client assessments and help develop an understanding of each client’s needs, preferences, goals, and home environment. Build strong client relationships through regular communication, follow-up, responsiveness, and a genuine commitment to their experience. Conduct new-hire orientations and help new caregivers understand Right at Home’s expectations, culture, policies, procedures, and commitment to quality care. Regularly review RightAssist reports, identify trends, concerns, opportunities, and follow-up items, and communicate findings to the appropriate members of the leadership team. Manage and support the Guide Program, ensuring clients and caregivers receive the appropriate guidance, resources, communication, and follow-through. Assist with HR activities, including onboarding, documentation, employee communication, and other administrative responsibilities as needed. Support caregiver engagement and accountability, helping caregivers feel connected, informed, and supported while maintaining high standards of care and professionalism. Identify and communicate changes in client needs, caregiver performance, scheduling concerns, service issues, or other matters that may impact the quality of care. Participate in client and caregiver follow-up, helping resolve issues before they become larger problems. Collaborate with the scheduling, operations, recruiting, and leadership teams to ensure everyone is working from the same information and toward the same goal. Help create a culture of exceptional service, where clients feel heard, caregivers feel valued, and families know they have a responsive team behind them. Represent Right at Home professionally with clients, families, caregivers, referral partners, and members of the community. Who You Are You’re someone who naturally builds relationships and enjoys helping people. You’re a strong communicator who can listen carefully, ask thoughtful questions, and handle difficult conversations with professionalism and empathy. You’re organized, proactive, and comfortable managing multiple priorities without losing sight of the people behind the paperwork, reports, and responsibilities. You notice details. You follow through. You don’t wait for someone else to solve a problem when you can take ownership and help make it better. You understand that exceptional home care depends on communication, consistency, accountability, and trust. You’re comfortable working with both people and processes—whether that means conducting a client assessment, orienting a new caregiver, reviewing a report, following up with a family, or assisting the HR team. Most importantly, you genuinely care about older adults and the families who trust us to help them remain safe, independent, and comfortable at home. Experience in home care, healthcare, senior services, care coordination, case management, human resources, nursing, social services, or another client-facing healthcare environment is highly valued. Home care experience is helpful, but not required. We’ll teach you our systems and our business. We’re looking for someone who already knows how to build trust, communicate effectively, take ownership, and make things happen.
