Description
The Referral Health Information Technician is a clinic specific role that ensures the timely delivery of medical records and processing of system referrals. The responsibilities include clear communication and education to clinical staff, strong attention to detail, and a knowledge base that supports the centralized processing of both these functions. The position will report directly to the Site Coordinator and include close collaboration with the centralized HIT team, Referrals team, and Operations Manager.
Document coordination

Daily am/pm distribution of documents and medical records. Coordinate all incoming and outgoing faxes to ensure proper timely distribution and in accordance to system procedures.

Manage interoffice mail daily to central HIT/Referrals office, including the logging of all documents transported between any Mosaic sites.

Ensure all documents meet proper signature requirements and contain 3 patient identifiers.

Maintain proper fax number at the clinic. Report incorrect documents received at the site and fax immediately to Central HIT upon receipt. Send a fax correction sheet to the site and ensure proper faxing.
ROI Coordination

Process site specific ROIs to ensure the appropriate information is collected prior to requesting records.

Create an Epic encounter and document ROI request.
Communication to the care teams

Educate and inform staff of HIT/Referrals processes. Act as a liaison for the site and central HIT to clarify and/or troubleshoot related issues.

Educate staff on appropriate documents approved to be scanned into the patient chart.
Referrals processing

Process all site specific urgent referrals.

Assist with system wide processing of referrals

Navigate and reference the CIMS website to identify current status of referrals and share with the appropriate care team members.

Track the status of referrals in collaboration of the centralized referrals team.

Act as a liaison for the site and central Referrals to clarify and/or trouble shoot related issues; and to provide pertinent referral information to the site and the central team.
Work Experience: One year of experience in a medical office setting. Referral coordination and health record management preferred. Experience working with low-income populations. Experience working on computers; typing/keyboarding speed of at least 40 WPM.
Education, certification and licensure: High school diploma or equivalency. Associates degree in related field, preferred.
Skills & Knowledge: Knowledge of standard office policies and procedures. Knowledge of standard office machines including copier, fax, shredder, multi-line telephone, printer, etc. Ability to manage multiple tasks and work independently using good judgment. Knowledge of Microsoft Office Software products and medical terminology preferred. Fluency in Spanish preferred.
