Job Summary:
The Chronic Care Management (CCM) Supervisor is responsible for the daily oversight, leadership, and coordination of Chronic Care Management Program. This position supervises Population Health Coordinator, CCM Specialists, CCM Navigators, and Community Health Workers (CHW) to ensure high-quality, patient-centered care, regulatory compliance, achievement of quality outcomes, and optimal reimbursement of CCM and related services. The CCM Supervisor provides clinical and operational guidance to staff, supports patient engagement initiatives, oversees care planning and documentation processes, ensures compliance with CMS and payer requirements, and serves as a liaison between care coordination staff, providers, and leadership.

Knowledge of medicine, public health, and health services issues

Working knowledge of standard desktop computing software packages (word processing, spread sheeting, presentation software, Internet browsers, etc.)

Familiarity with health care delivery in community health settings (preferred but not required)
Job Requirements:

Licensed Practical/Vocational Nurse; Associate’s degree in nursing or higher preferred. With an active and unrestricted NM or Compact State License

A minimum of 3 years of Clinical Health outcome experience; Care Coordination Management experience preferred

A minimum of 1 year of Supervisory experience
