Rate of Pay : $17.00 - $17.00 per hour

Community Habilitation Specialist

Part Time - Flexible days and hours - afternoons and early evenings,10 hours per week

Located throughout Erie County.

Valid New York State Driver’s License in good standing and use of personal vehicle for transporting individuals receiving services.

Mileage reimbursement for use of personal vehicle.

SUMMARY OF DUTIES

As a Community Habilitation Specialist, you will work directly with individuals with developmental/learning disabilities, providing services to the people we support so they can live independently and thrive in their communities. This could include, but is not limited to:

  • Community integration

  • Community travel and transportation

  • Money management and budgeting

  • Appropriate socialization/interactions

  • Healthy lifestyle skills/exercise

  • Medical appointments and understanding doctor’s orders

  • Additional skills needed to enable the individual to access the community and remain as independent as possible

  • Other instruction, services and supports as indicated by your supervisor.

SKILLS & ABILITIES

  • Possess good time management skills

  • Ability and desire to work as part of an interdisciplinary team

  • Ability to work 1-1 with an individual receiving services

  • Moderate level of written and oral communication skills

  • High level of interpersonal skills

  • Knowledge of behavioral techniques and developmental disabilities to assist in the care of individuals receiving services

  • Knowledge of computer and ability to utilize various software programs

  • Successful completion of First Aid, CPR, and Defensive Driving upon hire is required.

EDUCATION & TRAINING

Preferred: One year pertinent experience in health and human service related field and experience with individuals with disabilities

Minimum: High school diploma or equivalent
