Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes and optimizations. Maintain up-to-date patient/resident profiles, including medical history, lab results, and current medications (prescription, OTC, and home remedies). Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered, reviewed, and communicated to the healthcare team. Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support. Document all communications and clinical interventions in the patient's medical record, including interactions with families and caregivers. Implement and enforce policies and procedures for the safe handling, dispensing, and clinical oversight/management of medications.
