Pharmacist - LTC Role

Position Overview

The Pharmacist is responsible for reviewing and verifying medication orders, supporting safe and effective medication therapy, and providing clinical guidance to healthcare professionals. This role works closely with providers, nurses, and interdisciplinary teams to promote positive patient outcomes while ensuring compliance with all applicable pharmacy regulations and standards.

Key Responsibilities

  • Review and verify medication orders for accuracy, appropriateness, and safety

  • Evaluate drug therapies, dosing, interactions, contraindications, and potential adverse effects

  • Provide clinical recommendations and medication-related guidance to healthcare providers

  • Monitor medication therapy and support quality patient care initiatives

  • Collaborate with physicians, nurses, and other healthcare professionals

  • Maintain accurate patient medication records and clinical documentation

  • Support medication safety and regulatory compliance programs

  • Educate healthcare team members on medication-related best practices

  • Participate in quality assurance and continuous improvement efforts

Qualifications

  • Current Pharmacist license in the state of practice

  • PharmD preferred

  • At least one year of pharmacist experience preferred

  • Ability to work effectively within an interdisciplinary healthcare team

  • Experience with pharmacy software and electronic health record systems preferred

Schedule

  • Monday-Friday, 11:00 AM to 7:30 PM

  • Approximately one weekend per month, 9:00 AM to 5:30 PM
