Job Description

We are looking for a motivated and passionate part time (with benefits!) Physical Therapist to provide in-home care. All levels of experience are encouraged to apply and will train the right new graduate.

  • Job Type: Part time w/ benefits, 20-visits per week

  • Location: Bergen County (Including Fort Lee)

  • Schedule: Flexible!

Responsibilities:

  • Accept and treat patients within your designated geographic area

  • Provide 1:1 treatment

  • Evaluate patients and develop individualized treatment plans based on physician orders and patient needs

  • Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care

  • Educate family members, home health aides, and other caregivers on proper therapy techniques

  • Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.

  • Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.

  • Provide timely and accurate documentation for each visit

  • Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit

  • Must be willing to treat geriatric patients.

Qualifications

  • Physical Therapist Program Graduate

  • Licensed Physical Therapist in the state of New Jersey or eligible.

  • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.

  • Ability to travel to patients’ homes within the assigned geographic territory

  • Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.

The anticipated base salary range for this position is $60,000.00-$75,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Additionally, this position is eligible for incentive compensation. The Company's incentive compensation plan is subject to change. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
