Staff Pharmacist | Long-Term Care Pharmacy

We are seeking a licensed Pharmacist to join a collaborative pharmacy team supporting patients in long-term care and post-acute care environments. This opportunity offers a more clinical focus than traditional retail pharmacy, with an emphasis on medication therapy review, patient safety, and interdisciplinary collaboration.

What You'll Do

  • Review and verify medication orders for accuracy and clinical appropriateness

  • Evaluate dosing, drug interactions, and potential adverse effects

  • Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care

  • Monitor medication regimens and make clinical recommendations when appropriate

  • Support medication safety initiatives and regulatory compliance

  • Maintain accurate patient and pharmacy records

  • Participate in compounding and medication dispensing activities as needed
