Job Summary Works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services across the continuum of care. Maintains ethical principles and professional standards of practice in adherence to the discipline’s professional code of ethics. Job Responsibilities 1. Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate. 1. Collaborates with the interdisciplinary Team to assess for appropriate level of care and risk factors; develops and implements care/discharge plan and coordinates consults/referrals to remove barriers and promote optimal clinical and financial outcomes. 2. Utilizes specific therapeutic methods/strategies/interventions in accordance with individualized care plan. 3. Monitors progress of interventions and revises plan to meet patient/family needs. 4. Provides education to patient/family with respect to meeting established goals including but not limited to disease prevention, impact of illness and disease progression, advocacy for benefits, health maintenance, and adherence to treatment regimens. 2. Provides direct patient care that may include: case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end-of-life care, referral and discharge planning. 3. Refers patient to appropriate levels of care and available resources that promote the individualized care plan. 4. Maintains timely patient-centered documentation that reflect the patient and client systems’ pertinent information for assessment, interventions and outcomes in accordance with regulatory licensing requirements and third-party agencies. 5. Participates in administrative and development activities including team meetings, clinical supervision of staff and staff education. 6. Supports the development of hospital and/or unit-based programming to meet the clinical and strategic needs of the patients and the milieu. 7. Works as a team member to contribute to improved patient experience. 8.
