Job Summary and Responsibilities

As our Care Coordination Manager, you will provide operational leadership for the Care Coordination department in line with the CommonSpirit Health Care Coordination model, under the direction of the Director of Care Coordination.

Every day you will oversee clinical resource management, progression of care, discharge planning, patient advocacy, clinical social work, and care coordination services. You will supervise staff, manage workflow, patient progression, resource utilization, regulatory compliance, and quality outcomes, and partner with physicians, nursing, Utilization Management, and interdisciplinary teams to meet department priorities.

To be successful in this role, you will bring care coordination leadership experience, knowledge of regulatory requirements, and strong collaboration skills.

  • Provides operational leadership for the Care Coordination department through staff supervision, team development, coaching, and performance management. Coordinates and implements Care Coordination programs, initiatives, and workflows to support patient progression, resource utilization, quality outcomes, and organizational goals.

  • Manages daily department operations, including staffing assignments, workload balancing, scheduling, productivity, and resource allocation. Supervises front-line Care Coordination staff, including case managers, social workers, and support personnel, ensuring accountability for performance and service excellence.

  • Ensures timely completion of patient assessments, discharge planning, utilization management collaboration, documentation, and care transition activities in accordance with organizational standards. Ensures compliance with federal, state, accreditation, payer, and organizational requirements related to Care Coordination and Discharge Planning.

  • Monitors departmental performance metrics and operational data, identifies trends, implements corrective actions, and escalates significant issues to the Director. Implements departmental policies, procedures, standardized workflows, and process improvements to improve operational efficiency and patient outcomes.

  • Leads and/or participates in daily multidisciplinary rounds, progression of care activities, and barrier escalation processes to facilitate timely patient progression and safe transitions of care. Partners with physicians, nursing, Utilization Management, ancillary departments, and post-acute providers to resolve operational issues and improve coordination across the continuum of care.

  • Participates in multidisciplinary committees and performance improvement initiatives, implementing action plans to improve quality, patient experience, and operational outcomes. Supports regulatory readiness and accreditation activities by maintaining departmental compliance and ensuring required documentation and processes are followed.

Job Requirements

Required

  • Bachelors Of Science Bachelor's Degree in Nursing (e.g. BSN), upon hire or

  • Masters Other Master's Degree in Social Work, if not a Registered Nurse., upon hire or

  • Minimum of 3 years of clinical care coordination experience, (Utilization Management, Denial Management, Care Coordination) or 3 years of acute nursing team lead, charge or supervisory experience with working knowledge of care coordination and discharge planning., upon hire and

  • Operational experience, implementation, and staff development in healthcare environment, upon hire and

  • License required for Masters Social Worker.

  • Registered Nurse: CA, upon hire and

  • Certified Case Manager, within 12 - months or

  • Accredited Case Manager, within 12 - months

Preferred

  • Masters Other Master's Degree in Nursing, Healthcare Administration or related clinical field , upon hire and

  • Experience with performance improvement activities, upon hire

Pay Range

$52.27 - $77.75 /hour
