Description
Basic Function or Position Summary:
RPTXNon- exempt
Grade : 6
Reports to: HFS Director
18-20 hours/week $18.81 hourly
Responsible for conducting outreach and fostering trusting relationships with migrant
seasonal workers to provide assistance with preventive and primary health care and other
supportive services. Facilitates positive community connectivity.
$18.81 hourly 18-20 hours per week
Monday- Wednesday- Friday 9am-3 pm
Requirements
Position Responsibilities and Specific Duties:
- Provides outreach, education, advocacy, transportation, translation/interpretation, and
case management services to assist migrant and seasonal workers with access to
health care and supportive services.
- Provides intensive outreach in geographic areas served at places providing
employment for migrant seasonal workers.
- Conducts farm, home, or camp visits to foster trusting relationships, and assess
service needs.

Completes mobile intake.

Works with farmers and growers to facilitate access to services.

Facilitates ongoing community linkages for health, insurance, social, and other
supportive services.
- Provides ongoing visits, translations, transportation, advocacy, and service
coordination.

Maintains accurate and timely documentation.

Maintains regular vehicle maintenance schedule.

Accurate data entry into CAPTAIN and timely reporting.

Provides translation services as needed.

Assists with outreach and recruitment activities targeting the Hispanic population.

Promotes and assists with male involvement activities.

Attends mobile or onsite health clinics.

Promotes agency mission.
-
Maintains confidentiality and complies with the code of ethics.

Maintains consistent professional customer service.
-
Job performance incorporates integrated service delivery model while promoting self-
sufficiency.
- Other duties as designated by supervisor.
Key Working Relationships:
A. Internal: Health and Family Services Director, Fatherhood Coordinator, and
Fatherhood Staff.
B. External: Local farmers and growers, physicians and dentists, community agencies,
Chautauqua County Health Department, schools, churches, and customers.
Knowledge, Skills Required:

High School diploma or GED required. AA or AAS in Human Services preferred.

Experience with and knowledge of the migrant seasonal farm worker population preferred.

Strong verbal and written communication skills, including the ability to speak fluent Spanish.

Strong computer skills.

Knowledge of community resources in Chautauqua County preferred.
Special Requirements:

Must be able to provide consistent even tempered customer service at all times.

Bilingual English/Spanish required; English/Mixteco preferred.

Valid Driver’s license

Reliable transportation

Authorized Driver of a COI Vehicle.

Ability to travel throughout the counties served (Chautauqua, Erie, Niagara,
Cattaraugus and Allegany in the WNY Region)

Flexible work schedule

Pre-employment drug and alcohol testing required, subject to random drug and
alcohol testing for cause.
