In our Skilled Nursing Facility, recovery looks like getting back to the dining room without a walker, dressing independently again, or simply standing tall enough to hug a grandchild. As a Staff Physical Therapist, you'll get to know your residents over the course of their stay — not just their charts, but their goals, their stories, and what "home" means to them. You'll evaluate, build, and adjust individualized care plans, and be part of the team that gets residents there.

A Day in Your Life

? Evaluate newly admitted residents and build a roadmap for recovery — balancing short-term rehab goals with long-term quality-of-life needs.

? Put your hands-on skills to work with electrical stimulation, ultrasound, hydrotherapy, therapeutic exercise, gait and mobility training, and mobilization tailored to older adults.

? Coach residents and families through home exercise programs and discharge planning so progress continues, whether that's home or a lower level of care.

? Team up daily with Occupational and Speech Therapists, nursing staff, and physicians in a true interdisciplinary SNF care model.

? Guide and mentor your Physical Therapy Assistants and Aide, and help train the next generation as a clinical instructor for PT students.

? Step into the "In-Charge" role when needed and help shape department policy — real growth, real ownership.

? Rotate through required NYS regulatory evaluations for newly admitted nursing home residents, keeping every resident's care plan compliant and current.

The Essentials, At a Glance

• Evaluate residents and build individualized care plans, including compensation forms as required.

• Administer treatments safely, explaining each step to the resident and checking in throughout.

• Write timely, accurate evaluations, progress notes, and discharge summaries.

• Maintain equipment and follow infection-control and universal precautions.

• Keep the referring physician and care team in the loop on resident progress, adjusting the care plan to hit established goals.

• Maintain a caseload of SNF residents as assigned by the department manager.

• Evaluate newly admitted nursing home residents per NYS regulation, on a rotating basis among SNF Staff Physical Therapists.

• Provide treatments and evaluations in the nursing home, including occasional weekends, as caseload requires.

• Uphold resident confidentiality at every step.

This role is classified as Medium Work: lifting up to 50 lbs, frequent carrying up to 25 lbs, and close visual attention roughly half the day. Reasonable accommodations are available.

Requirements

What You Bring

• A Bachelor of Science in Physical Therapy (Master's degree a plus).

• A current New York State Physical Therapist License.

• Working knowledge of NY State and federal PT regulations, including SNF/long-term care requirements.

• Prior experience in a rehabilitative medicine or skilled nursing setting preferred, but not required.

• Sharp clinical judgment — the ability to read a resident's condition, adapt a treatment plan, and know when something needs a second look.

• Genuine comfort and patience working with a geriatric population, including residents with cognitive or communication challenges.

• Clear communication skills, written and verbal, for charting, resident/family education, and physician updates.

• Physical stamina: this is active, on-your-feet work that includes lifting, transferring, and hands-on treatment.
