Join DaVita in Revolutionizing Care for ESKD Patients: Advanced Practice Provider (NP/PA) Opportunity in the Virginia Beach/'Norfolk Market
DaVita is pioneering a new approach to holistic care for patients with end-stage renal disease (ESKD) directly within our dialysis centers. We're seeking a highly motivated and innovative Advanced Practice Provider (NP/PA) – Nurse Practitioner or Physician Assistant – to deliver wrap-around care chairside to a high-need population, helping us build and shape this groundbreaking program.
ESKD patients are affected by more than just kidney disease – they almost always present with a consistent constellation of cardiovascular, kidney, and metabolic diseases, also referred to as CKM. It is estimated that 30-50% of patients with renal failure do not receive consistent primary care support, resulting in increased morbidity, hospitalizations, and mortality. This program will address the care barriers kidney patients face—an absence of care for the whole patient, leading to hospitalizations and mortality.
Our Goal
- Delivering care to the patient: Patients receive care in the health setting they spend the most time in: the dialysis unit. No additional patient transportation is required.
- Fostering seamless coordination: Proximity to and coordination with other critical caretakers, such as the Nephrologist, dialysis teammates, and integrated care leaders. Many of the most critical care takers, including the nephrologist and dialysis team, are in the dialysis clinic. By working out of the clinic, you work as a true interdisciplinary team.
The Opportunity
Starting a program requires flexibility and patience, but it affords the opportunity to play a pivotal role in defining the initiative’s clinical direction. You will have direct access to top DaVita leaders to provide feedback on innovation and care redesign. It also means that, pending performance, there will be future leadership opportunities as we scale the program. We are looking for someone who does not simply manage disease but actively looks for opportunities to improve care delivery and outcomes.
Role Specifics
- Location: You will work from a portfolio of 3–8 DaVita dialysis centers within the Virginia Beach, VA market.
- Hours: Your schedule will revolve around the time patients will come into the dialysis units, primarily between 8:00 AM and 5:00 PM. However, some days may require you to come earlier or stay later if the dialysis patient has an early or later treatment time.
- Autonomy: You will have clinical autonomy to determine which patients to see and during which time.
Expectations
- Deliver care chairside during dialysis treatments, minimizing disruption to the patient’s dialysis schedule.
- Average 10–12 patient visits per day.
- Coordinate patient follow-ups to proactively address patient concerns, including coordinating care with other providers, ordering key labs, and prescribing medication.
- Build clinical relationships and collaborate closely with nephrologists, dialysis teammates, integrated care teammates, and local PCPs and specialists.
- Conduct annual comprehensive health evaluations.
- Provide comprehensive CKM disease management with an emphasis on prevention and early intervention.
- Identify clinical deterioration early and intervene before conditions escalate to an emergency department visit or hospitalization.
- Support transitions of care following hospitalizations and work to prevent avoidable readmissions.
- Utilize population health principles, clinical data, and evidence-based guidelines to proactively manage high-risk patients.
Qualifications
- 3+ years of independent primary care practice
- License to practice autonomously
- DEA license and comfort in prescribing for chronic diseases typically associated with ESRD, such as congestive heart failure.
- Strong clinical assessment and diagnostic skills.
- Experience caring for medically complex adult patients.
- Excellent communication and interpersonal skills.
- Ability to function independently while working within a highly collaborative interdisciplinary team.
- Experience with dialysis or nephrology (preferred, but not required)
Preferred Qualifications
- Minimum of 5+ years of clinical experience in Primary Care, Internal Medicine, or Gerontology.
- Demonstrated expertise in the management of chronic health conditions, including but not limited to diabetes, hypertension, chronic respiratory diseases (e.g., COPD), and heart disease.
- Proven experience in prescribing and managing medications: ability to initiate, titrate, and monitor therapies such as insulin, antihypertensives, statins, and other evidence-based treatments aligned with current clinical guidelines.

The Salary Range for the role is $133,000.00 - $164,000.00 per year.
