This is a rewarding opportunity to provide comprehensive, patient-centered care in a well-established outpatient setting.

Responsibilities

Position Overview:• Full-time position in an outpatient primary care clinic• See patients for preventive care, chronic disease management, and acute visits• Collaborate closely with a team of physicians, APPs, and clinical support staff• Manage your own patient panel with opportunities for continuity of care• 40 hour/week (36 patient hours/4 admin hours) schedule with minimal call responsibilityWhat We Offer:• Competitive compensation and comprehensive benefits package• Supportive, team-based environment with leadership committed to APP development• CME support and professional growth opportunities• Mission-driven organization with a strong focus on community health

Qualifications

• Licensed and certified as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Illinois

• Board-certified in Family Medicine, Adult-Gerontology, or related field

• Strong clinical assessment, communication, and documentation skills• Commitment to providing high-quality, compassionate care

Prime Healthcare Illinois Medical Group offers competitive compensation and a comprehensive benefits package that provides employees the flexibility to tailor benefits according to their individual needs. A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $115,000.00 to $130,000.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
