Full job description

Under the direction of the Director of Social Work, this clinician will provide assessment, advocacy and discharge planning for the psycho-social needs of the patients of all ages. This individual will need to demonstrate expertise in crisis intervention, mandated reporting, care coordination of patients with psychosocial risk. Responsible for education and advocacy of patients, families and/or significant others. Knowledge of community resources is required. Must possess a Master's Degree in Social Work and be licensed, or license eligible, as a LCSW in the state of Massachusetts. Must be able to prioritize multiple tasks, manage high patient volume and possess strong interpersonal skills. Must have excellent communication skills and display a professional demeanor with patients, families, interdisciplinary team members, and outside agencies. 2-5 years of acute hospital experience preferred.
